Dear Mr. Editor's Name:

May I interest you in reading MANUSCRIPT TITLE IN CAPS, my [middle grade chapter book] manuscript?  It is a work of [contemporary fiction for ages 8-12 or whatever it is.] We met at [conference or other place where you met, even briefly, or some explanation of why you are sending your manuscript to this particular editor. Tip: If you haven't met, study the latest book list from this publisher and say that you've studied their list and explain why you think your book is a good fit for their list.]

[A one paragraph synopsis goes here.  Start with a good hook, then give the editor an indication of where the story is going.  Give them beginning, middle and end.]

I have previously published in [say where you are published, or note upcoming publications.  If not published, state relevant affiliations, such as SCBWI memberships.  Keep it very brief.]

Thank you for your time and consideration.  Per the submissions instructions on your website, I have enclosed [three sample chapters, a one-page synopsis and a SASE for your reply.] I look forward to your reply.

Sincerely,
